Unified Communications and Surveillance in the Clinical Setting

For the modern healthcare delivery model to operate efficiently there is an undeniable mandate for seamless, comprehensive and real-time communications across the continuum of care. In this paper we will look at the link and need for collaboration and clinical information amongst various stakeholders from acute care hospitals, primary care physicians, long-term and sub-acute care providers, and to patient’s family care providers to ensure they receive the right care at the right time.
